Your health plan will only pay for services that it determines reasonable and necessary. These services are outlined in the explanations of benefits provided by your insurance carrier under your benefit plan. If your health plan determines that a particular service, although deemed important to you by your Physician and yourself, is not reasonable and necessary under your health plan standards, your health plan will deny payment for that service. We are not a provider for any insurance. We do not bill insurance. We do accept HSA charge cards.
In order to offer care to all of those in need, patients are asked to provide 24 hours notice if they are unable to keep their appointment so that we may offer it to other patients. We do understand that occasionally a patient may not be able to avoid canceling appointments. However, if appointments are frequently missed or canceled, we will work directly with patients to resolve the issue. We reserve the right to charge for missed appointments at our discretion and/or suspend treatment.
We accept cash, checks and credit cards as forms of payments. A $35.00 fee will be assessed on all returned checks. We do not provide refunds for any supplements or lab kits.
